A 28-year-old woman at 10 weeks’ gestation with a previously confirmed intrauterine pregnancy presents with 12 hours of worsening right lower quadrant abdominal pain, anorexia, and vomiting. Temperature is 38.2°C, heart rate is 108/min, and blood pressure is 112/70 mm Hg. Exam shows focal right lower quadrant tenderness with guarding. WBC count is 15,800/µL. Which problem representation best frames the initial differential and next step?
Problem Representation Framework
Appendiceal obstruction causes initially visceral pain that localizes with peritoneal inflammation; fever, leukocytosis, focal guarding, and pregnancy modifier make C the best representation and management frame. C is tempting because she is pregnant, but confirmed intrauterine pregnancy and no bleeding/adnexal findings argue against ectopic pregnancy. A shares pelvic pain and fever, but PID usually has cervical motion tenderness or discharge and is less likely in pregnancy. A shares vomiting and fever, but gastroenteritis typically causes diffuse cramping and diarrhea without focal peritonitis. Review Problem Representation Framework — focus on acuity, localization, discriminators, and severity modifiers.
Diagnose suspected appendicitis; order graded-compression ultrasound then MRI if nondiagnostic; treat with NPO, IV fluids, antibiotics; escalate urgently to surgery/OB.
