A pharmacist is verifying medications for Ms. L., age 84 years, weight 58 kg, SCr 1.6 mg/dL. Her profile includes apixaban 5 mg PO twice daily for atrial fibrillation. A new prescription is clarithromycin 500 mg PO twice daily for 7 days. Which medication-related problem and pharmacist action should be prioritized?
C is correct: apixaban exposure rises with strong CYP3A4/P-gp inhibition; this patient also meets NVAF dose-reduction criteria, so clarify before dispensing. D is tempting as counseling, but spacing does not remove metabolic/transport inhibition. B is tempting for adherence, but rivaroxaban is also affected by strong inhibitors and requires renal review. A uses the right dose criterion but misses the interacting clarithromycin and need to contact the prescriber.

Learners
What Regulated Science Institute learners say about the platform (reviews across all exams, not only NAPLEX).
The guided drills were the difference. I stopped re-reading notes and started doing timed sets. My scores jumped in two weeks.
The explanations are crisp and the pacing is perfect. I used the active recall prompts every night and it stuck.
I loved the frameworks and examples. Prioritization became obvious, and the drills fixed my weak spots fast.
